Park Place Transitional Care and Rehabilitation is a 106 bed Medicare Certified skilled nursing facility. We are located in the beautiful and serene city of Great Falls, Montana. With stunning views of the mountains, a quaint park just a short walk away, and lovely gardens on the grounds, this is the ideal place to start your healing journey.
We provide 24/7 nursing care, physical therapy, occupational therapy, and speech therapy. With an array of daily activities, staff that care, and amazing food our goal is simple – provide the best quality of care imaginable in a place that feel like home. Often the transition to a skilled nursing and rehabilitation facility is challenging. At Park Place Transitional Care, we strive to ease the process by providing an environment where residents and guests feel informed and comfortable.
In addition to nurses and staff who are always available to answer questions and lend a hand, each resident is paired with a coordinator, to help ease the transition and provide a familiar smiling face.
Our team of qualified professionals helps residents recover from surgery, injury or serious illness. We understand the importance of creating a comfortable and nurturing atmosphere, whether residents stay for short-term treatment or long-term care.
